It’s Flu Season!
WHMS’s scheduled FLU CLINIC is Monday, September 28, 2026
starting at 8:30 a.m.
Flu forms will be sent home on September 9, 2026.
Only the FLU Vaccine will be given on this day.
PLEASE:
Fill out the FRONT AND BACK of form included in packet
Fill out the FERPA form also included in packet
RETURN BOTH FORMS by Monday, September 21, 2026
Your child cannot be vaccinated without BOTH forms
Forms WILL NOT be accepted on the day of clinic. The Health Department will only bring the
number of shots that I provide on the cutoff date. I DO encourage you to have your child
vaccinated. There will NOT be any Flu Mist this year.
