All students with shots currently not up to date, must turn in documentation as soon as possible after receiving the vaccine or making an appointment to do so. If you choose to not vaccinate your child, an exemption letter from the ADH must be acquired and renewed yearly. All students that are noncompliant will be be dismissed from school with unexcused absences until documentation is provided to the health office.
Any new or transfer students please come by the health office to fill out any necessary health information or permission forms as soon as possible. Please turn a complete immunization to the health office as well.
If your child has medication in my office to be administered during school hours, please make sure all paperwork including a health care plan from your child's physician is turned in as soon as possible, if you haven't already done so this school year. This needs to be updated yearly. The medications must be brought to the office and picked up at the end of the year by the parent.
If you received results in the mail from your child's hearing and/or vision screen, please remember that I need documentation from a physician with in 60 days of receiving the notice. If you did not receive results then your child passed their screenings.
