7th Grade Requirements

• TDaP--This is a NEW REQUIREMENT--all students entering the 7th grade must have an up to date tetanus with a pertussis booster (TDaP).  Must have had this within the last two (2) years.
• Hepatitis B--This is a 3 shot series, all 3 shots are required to be in compliance.
• Varicella (Chicken Pox)--Students now have to be vaccinated against the chicken pox.  If your child has had the disease we must have documentation from a DOCTOR stating so, or a statement in writing by a parent or guardian that the student has had the disease.

These requirements must be met when your child enters the 7th grade!!
